To apply for the Ayushman Bharat health card, visit the self-service portal. Here’s a step-by-step process:Enter your mobile number and click on the verification option. An OTP will be sent to your mobile phone. Enter this OTP in the designated box and log in.Choose PM-JAY (Pradhan Mantri Jan Arogya Yojana) from the scheme options and select your state.You can search using various options such as family ID, Aadhaar number, name, or location. Here, we will use the Aadhaar number.: Input your 12-digit Aadhaar number and search. Your health card details should appear if the information matches.

Many people face the issue of their Ayushman card being in a pending state and not being able to download it. Here’s how to resolve this:Search for PM-JAY on your browser. Click on the nia.gov.in option to access the official portal.On the portal, look for the option to register your governance issues. Select PM-JAY from the governance type section.Provide necessary details such as your name, gender, date of birth, mobile number, state, district, and complete address. Select the complaint type (inquiry or request) and mention that your Ayushman card is pending.Attach any necessary ID proofs or documents. If you encounter issues with file sizes, use an online tool to compress or convert your files to PDF format.After filling in all details and attaching documents, submit the form. An OTP will be sent to your mobile for verification. Enter the OTP to complete the process.
Tracking Your Complaint
To track the status of your complaint:Use the “Track Your Governance” option on the portal. Enter the YGN number provided on your acknowledgment slip.An OTP will be sent for verification. Enter the OTP to view the current status of your complaint.
